I have been reading books on a long list that I never got to but must have--at one time--thought I would like to. I am pleased with my younger self's "to read list." The Demons by Dostoevsky never read, now read. It is a prophecy.

nobody ever said the last part of the journey can't be as fascinating as the beginning or middle. I wished your skills for improving services had trickled down to our Canadian healthcare system. A monopoly third party payment system could use the customer advocates you spoke of. Thanks for sharing

Canada's healthcare system claims to have customer advocates, called case managers. Case managers typically aren't only customer advocates. They may also coordinate care and work to find less expensive ways to achieve the same result. In institutions they can also be outcry witnesses to sexual assault. This is nowhere near a complete job description.

A patient advocate has a tough role, especially in institutional settings. The position is paid by the same organization as the care provider. This creates the potential for a conflict of interests. After retiring from practice-centered work as a clinical medical assistant, my wife joined the volunteer group at our nearest hospital. She had a number of assignments, including independent patient advocate. She had no conflict of interests, which made her more effective.

By the way, I subscribe to a Youtube channel, "Medicine with Dr. Moran," a Canadian physician with a real knack for plain-language explanations of medical issues.

Ontario Patient Ombudsmen is a thin veneer of patient advocacy. By design they basically have no teeth. For effective advocacy the advocate needs to be independent from the service delivering the care. In my over 30 years of nursing I have seen many systemic abuses rooted into the fact that there is no competition for service. Thank you for the Youtube channel.

You're welcome. I messaged him that I've been non-practicing for half of forever, and his review of stuff I learned in med school was as valuable as the new stuff he goes over. Dr. Moran is a gem.
